Home Management Programme

Overview The Home Management Programme (HMP) specialises in providing comprehensive therapy and intervention services to individuals with cerebral palsy and associated disabilities, for better social and community engagement. Utilising an interdisciplinary approach, our registered therapists offer a variety of therapies, including physiotherapy, occupational therapy, and speech-language therapy directly at the client's residence for maximized convenience and efficacy. Core Services We offer an extensive range of services tailored to meet individual needs:
  • Assessment: Initial evaluations to gauge the client’s specific conditions and requirements.
  • Goal Planning: Crafting of a personalised treatment regimen focused on achieving targeted outcomes.
  • Intervention: Execution of the prescribed therapies and interventions.
  • Monitoring: Ongoing assessment of client's progress throughout the treatment.
  • Review: Periodic re-evaluations to adjust treatment protocols as needed.
  • Equipment Recommendation: Suggesting suitable assistive devices for enhanced functionality.
  • Caregiver Training: Providing essential training to caregivers for effective home-based support.
  • Self-Care for Caregivers: Strategies and resources for caregiver well-being.
Domains of Intervention Our interventions concentrate on the improvement of:
  • Fine and gross motor skills
  • Cognitive and perceptual abilities
  • Communication skills
  • Safe eating, drinking, and swallowing skills
  • Social engagement
  • Self-help capabilities, including the adoption and effective use of assistive technology
Admission Criteria To be eligible for enrolment into the programme, clients must:
  • Have a diagnosis of cerebral palsy or related developmental disabilities, confirmed by a certified medical practitioner.
  • Be Singapore Citizens or Permanent Residents, irrespective of race, language, or religion.
  • Be aged between 7 and 60 years.
  • Obtain parental or guardian consent to comply with the terms outlined in our Therapy Rehabilitation Programme.
  • Submit a medical report specifying the diagnosed disability.
Operating Hours Our therapy services are accessible for home visits from 8 a.m. to 5 p.m., Monday to Friday. Duration and Frequency Each session lasts between 45 minutes to an hour, with sessions scheduled once every 5 weeks based on individual needs assessments. Fees and Subsidies
