TRANS-DISCIPLINARY (TRANS-D) APPROACH

Transdisciplinary Integrated Collaborative Care (Trans-D ICC) Approach

The Trans-D ICC approach is a team-based way of working where professionals and families come together to support a child’s learning and development. It focuses on reducing caregiver stress and improving outcomes for both the child and family by creating learning opportunities in everyday routines and settings.

 

Goals of the Trans-D Approach

  • Build strong teamwork among therapists, teachers, and parents
  • Deliver services in a smooth and coordinated way
  • Create learning opportunities across different environments
  • Support families by coaching them to include learning in daily routines and offering emotional support when needed

 

How the Team Works

The Trans-D team includes the class teacher or interventionist and allied health professionals such as:
  • Occupational Therapist
  • Physiotherapist
  • Speech and Language Therapist
  • Social Worker
  • Psychologist
They meet regularly to plan and carry out individualised learning programs together. The teacher, often the key coordinator, works closely with the allied health team to design lessons that include individual goals. Each team member contributes their expertise and supports/coaches one another—and the family—for successful implementation.

 

Why It Matters

This approach helps children learn in natural, everyday learning and routine settings and ensures families feel supported and empowered. It creates a clear path to high-quality, meaningful learning experiences thereby leading to better client and family outcomes.
